AHD - Advance Healthcare Directive

These are declarations provided by citizens regarding health treatment to be received or refused in cases of incapacity.

 The Civil Registry Office is authorised to receive only those AHDs drawn up and submitted personally by the person making the declaration, and may not provide medical information or advice on the content of the declaration itself, nor may the person making the declaration participate in the drafting of the declaration. The necessary information can be obtained from the local social and health unit or from one’s doctor.

 In the declaration, the person making it may designate a trustee to act on his or her behalf and represent him or her in relations between doctor and patient. Acceptance of the appointment of the trustee is made by signing the AHD or by a subsequent deed. The trustee may renounce the appointment in writing communicated to the person making the declaration and the latter may revoke the trustee's appointment at any time.

 AHDs are renewable, modifiable and revocable at any time, in the same form in which they were drawn up.

 For whom is it intended?

To any person of legal age and of sound mind residing in the municipality of Venice

 What to do 

AHDs must be drawn up:

- by public notarial deed;

- by private deed authenticated by a notary;

- or by private deed delivered, in original form, by the person making the declaration, to the Civil Registry Office of the municipality where the person making the declaration resides.

 The following documents must be presented to the Civil Registry Office:

  • Original AHD and photocopies thereof (one for each trustee);

  • valid identification document of the person making the declaration and photocopy thereof;

  • photocopy of the identity document of the trustee(s).

There is no charge to users.

 Quality Standards

Time interval between request and registration of the declaration <= 30 days.
